客家人群同型半胱氨酸与冠心病的相关性研究(1)

     【摘要】 目的:探究客家人群同型半胱氨酸(homocysteine,Hcy)与冠心病(coronary artery heart disease,CHD)的相关性。方法:选取2015年1月-2016年12月本院收治的CHD患者96例为观察组,根据不同治疗方式将观察组分为常规治疗组(常规治疗)和降Hcy治疗组(常规治疗+叶酸+维生素B12+维生素B6),各48例。另选取本院同期健康体检者74例为对照组。比较观察组和对照组的Hcy、TC、TG、VB12、FA、HDL-C、LDL-C水平,分析CHD患者的危险因素,统计CHD患者12个月内的复发情况。结果:观察组年龄、Hcy、TC、TG水平均高于对照组,VB12、FA、HDL-C水平均低于对照组,比较差异均有统计学意义(P<0.05);年龄、TG、Hcy、HDL-C、VB12、FA为CHD发生的危险因素(P<0.05);降Hcy治疗组复发率为16.67%,明显低于常规治疗组的35.42%,比较差异有统计学意义(字2=4.381,P=0.036)。结论:Hcy可作为CHD的独立危险因素,对临床CHD的预防、治疗及监测具有积极意义。

    【Abstract】 Objective:To study the correlation between homocysteine(Hcy) and coronary heart disease(CHD) in Hakka population.Method:A total of 96 patients with CHD admitted in our hospital from January 2015 to December 2016 were selected as observation group,according to different treatment methods,they were divided into routine treatment group(routine treatment) and lower Hcy treatment group(routine treatment+folic acid+vitamin B12+vitamin B6),48 cases in each group.In addition,74 cases of health examination in the same period were selected as control group.The levels of Hcy,TC,TG,VB12,FA,HDL-C and LDL-C were compared between observation group and control group,the risk factors of CHD patients were analyzed,and the recurrence of CHD patients in 12 months was statistically analyzed.Result:The levels of age,Hcy,TC and TG in observation group were all higher than those of control group,the levels of VB12,FA and HDL-C were all lower than those of control group,the differences were statistically significant(P<0.05).The risk factors of CHD were age,TG,Hcy,HDL-C,VB12 and FA(P<0.05).The recurrence rate in lower Hcy treatment group was 16.67%,which was significantly lower than 35.42% of routine treatment group,the difference was statistically significant(字2=4.381,P=0.036).Conclusion:Hcy can be an independent risk factor for CHD,and has positive meaning for Prevention,treatment and monitoring of CHD.

    現今社会,人口老龄化随着生活水平的提高而持续加剧。老年患者冠心病(coronary artery heart disease,CHD)发病率较高,已成为影响健康的主要疾病。我国目前CHD患者正以每年20%的速度增加,死亡率占所有心脏病死亡人数的10%~20%[1]。如何提早发现、预防并控制CHD已成为人们日益关注的重点问题。血压、血脂、血糖、血尿酸偏高均为CHD的高危因素[2]。此类患者因饮食习惯(嗜咸、嗜油)类似,往往出现在客家人群中。因此,客家人群CHD存在较高发生率,对于客家人群CHD的发现和预防也应引起足够重视[3]。研究显示,同型半胱氨酸(homocysteine,Hcy)与CHD关系密切,高Hcy血症或成为CHD独立的危险因素[4]。随着Hcy深入研究,对预测CHD及提前预防具有重要意义。本次研究旨在探讨客家人群中CHD与Hcy的相关性,现报道如下。, 百拇医药(陈志龙 钟凯 何宇东)
